What do you know about sedation dentistry?

Due to prior torture at the hands of dentists when I was a child, I am scared to death of going to the dentist. I know that I really need to go and I’ve seen some commercials for “pain-free” and “worry free” dentists. From what I gather, they put you under conscious sedation (not exactly sure what that is either) and do all the dental work while you are basically asleep. Opinions?

Sedation dentistry and periodontal disease?

I think I might have periodontal disease, and I’m freaking out. I have an extreme phobia of the dentist, and haven’t been in about 12 years! I assume I’ll have to get a deep cleaning/scaling/planing done, and I am terrified. I have been considering sedation dentistry, the asleep kind of sedation (iv), not the conscious sedation (laughing gas), although I admit I’m a little weary of being completely knocked out. Anyone have any experience with any of this?

Assistant answers:

IV Sedation at the dentist is NOT being knocked out. You think you are, but you aren’t. After they give the IV you feel like you just fell asleep and woke up instantly in a recovery room but that’s just how you feel. You’re actually awake and aware of what they are doing during the entire procedure but remember absolutely nothing about it.

A few questions about sedation dentistry?

I’m going to need a gum graft but just got a frenectomy done so my periodontist hasn’t talked to me about the gum graft yet. But I DEFINITELY don’t want to go through with the gum graft without a sedative…even just for the frenectomy (which was done with a laser and finished in less than a minute), I was bawling for 10 minutes before he could actually do the procedure. I am SUCH a baby when it comes to surgery…

My mind is VERY convincing and honestly I’m worried that a Valium won’t be enough to calm me down. I doubt I’ll pass out from it, and my mind has sort of an “anti-placebo” effect…I get so worried it won’t work and therefore it doesn’t. I used to do drugs (not for a long time), but I would take 5 or 6 ecstasy and convince myself it wasn’t working and not feel it. Same with Xanax, I could take 2 or 3 and convince myself nothing was working.

So I’m worried that I’ll still be freaked out even if I get a Valium…but I’m pretty sure they only operate on conscious patients….do you think there’s any solution for me? Or I’ll just have to go through with it freaked out?

I can understand where you’re coming from too. I’m the same way. The last time I went into the oral surgeon for something I was sent with Valium, it made me awfully loopy and silly, but it still wasn’t enough to make me not care what was going on.

I was very happy that they also gave me nitrous oxide, both together worked quite well. You should see if they can give you the gas or both it’s much more comfortable.

I’ve also heard people taking laughing gas and still being awake and feeling pain. I took 1mg of xanax the other night and it didnt do much. What should I do? Just find a dentist with laughing gas, laser dentistry, and just in case IV sedation for a cavity filling? I cannot see a needle going towards my mouth, not as long as I am somewhat conscious. Please help… this is from bad childhood experience with dentists.

Assistant answers:

Laser dentistry is an option for some. depending on how deep your cavity is, you may need to be numbed up still. if this is an interest to you, call around to different dentists to see who does laser dentistry. just a warning, you may be paying more out of pocket to have it done with a laser. nitrous oxide, laughing gas, will not put you to sleep. it is a mild form of sedation meaning you will feel a sort of high. you will be aware of everything. if your dental/needle phobia is this extreme, I do not recommend you use nitrous alone with out other medications to accompany as given to you by your dentist. if none of this helps, find a dentist in your area that does IV sedation. do whatever it takes before your simple fillings turn into root canals.

Assistant answers:

Implants are expensive, that’s for sure….

Dental insurance companies don’t usually cover any of the fee for implants, but they do cover some of the cost of the crown that’s put over/on it.

Beware if/when you do get dental insurance, some companies have a mandatory waiting period for prosthetic work…

If you’re eligible, AARP has a decent plan with Delta Dental, and the premiums are decent.

You should probably plan to be at the dentist for 1 and a1/2 to 2 hours to be safe. Not that you will be sedated for that time but there will be prep / waiting before they put you out and then recovery after you come to. The actual procedure itself – once you are knocked out and relaxed – should take less than an hour. Your dentist should be able to tell you how long you will be under for in relation to your insurance.

IV sedation is very safe. You are always breathing on your own and are responsive. There are no lasting complications. Laser caries removal is not always possible on some tooth procedures. You need to relax and not worry so much. Good luck with your dental work and you’ll do just fine.

How much would cosmetic dentistry cost me?

I have a little “snaggle” tooth, basically a sharp pointy tooth that is shorter than all my other teeth. I HATE it. It’s not horrible or anything, but i never fully smile because I hate it. How much would it cost me to get that fixed (about). Also, a lot of my other teeth are kind of sharp and pointy too, can a dentist just file those so they look more square??
By the way, I have dental insurance, not sure if that would make any difference

Assistant answers:

If you just need that one tooth fixed, especially if you have dental insurance, it may not be too much to fix. Based on your description, you probably need a crown, or possibly just a veneer. Either of those can be made to look almost exactly like your other teeth.

If you do get a crown, full porcelain (if it’s a front tooth) or Lava crowns are the most esthetic.

Veneers are normally porcelain, so that would look very nice as well if it is an option.

As for cost, depending on where you live and how much your insurance covers, it may be anywhere from a $3-400 up to ~$1000. The previous person I believe is talking about an implant, which is much more expensive than a single crown.

Regarding your other teeth, your dentist would have to determine whether that could be done, but normally it is not recommended to “file” your teeth too much, because once the enamel is removed, there is no turning back!

They may be able to add some composite/tooth colored filling material in order to achieve the look you desire, which is essentially reversible.

Hope that helps.

Full Mouth Reconstruction or Full Mouth Rehabilitation refers to the process of restoration and alteration of every single existing or missing tooth position in the mouth.

The existing teeth are usually restored for crowns while missing teeth positions are prepared to have dental implants or fixed dental bridges.

Both dental implants and fixed bridges are permanent fixed tooth replacement options. Many dentists will tell you that a dental implant is a much better option and that is true. To explain why this is true we would need an additional article for that.

In the case where there are no teeth missing, Full Mouth Reconstruction is similar to Full Mouth Crowns because every single tooth would be fully covered with a crown. Full Mouth Cosmetic Reconstruction is also termed as Full Mouth Veneers or Full Mouth Dental Veneers by most. In reality we dentists refrain from veneering every single tooth in the mouth. Generally the teeth that are visible within the smile zone are indicated for dental veneers. In situations where all the teeth are missing and implants with fixed restorations are the restorative option, this process refers to Full Mouth Implants or Full Mouth Dental Implants.

Therefore Full Mouth Reconstruction can involve dental veneers, crowns, dental implants, fixed dental bridges or any combination of any of these restorative options. In any case, all existing teeth are altered and thus the bite dynamics and mechanics can be altered.

The most common reasons for Full Mouth Reconstruction or Full Mouth Rehabilitation are:

1- Missing teeth:

When all or most of the teeth are missing to the point that the existing bite is inadequate, Full Mouth Reconstruction can be a restorative option.

2- Severe wear of tooth structure:

Sometimes long term strong grinding habits can lead to wear and loss of up to 50% or more of the structure of teeth. In this case the original bite is where the patient tends to over close to force the teeth to touch since most of the teeth height is reduced and missing due to wear. In this case Full Mouth Reconstruction can certainly bring the bite back to the original height and restore the bite mechanics to a much more favorable situation.

3- Severe Decay or Cavities:

When all or most of the teeth in the mouth are severely decayed to the point where most of the tooth structure is affected, Full Mouth Reconstruction is indicted where the teeth are restored with full mouth crowns.

4- Bite Correction:

When the bite correction attempts have been exhausted through orthodontic means and when the orthodontist feels that further bite correction is required but can only accomplished with restorative means, Full Mouth Reconstruction can be an option.

This summarizes the major reasons for Full Mouth Rehabilitation or Full Mouth Reconstruction.

When choosing a dentist for Full Mouth Reconstruction, second opinions are important. The experience of dentists and level of clinical skill varies, just like most professions. It is important that you choose a dentist who possesses a deep understanding of bite mechanics and dental aesthetics. Bite complication is the most common complication after Full Mouth Reconstruction, generally due to inadequate bite dynamics.

Locally, Full Mouth Reconstruction Portland, there are several highly qualified dentists who are well capable of adequately restoring the entire mouth. At places like Dental Dynamics (a general dentist in Portland, OR) a thorough consultation and analysis of the current bite is conducted and the entire mouth is restored with custom lab made temporary restorations that look and mimic the final restoration. These temporary restorations are worn for 2-3 months to make sure the bite mechanics are adequate for the long run.

When making such a large investment toward your oral health, you want to make sure that your dentist stands behind his or her work. Make sure that there is some kind of warranty on the dental work.Sometimes you’ll even get lucky at places like Dental Dynamics, Full Mouth Rehabilitation cases have a five year warranty.

Dental implants Portland and surrounding areas are changing lives of edentulous patients, one patient at a time. The predictability of successful dental implant surgery is better than ever with the introduction of Cone Beam CT Scan “CBCT” technology. This technology paved the way for computer guided dental implant surgery.

One of the first Cone beam CT scans has been introduced to Portland, Oregon by Dental Dynamics PC. Dental Dynamics is a local general dental practice focusing on providing the latest in dental innovation and technology in the areas of implant dentistry, cosmetic dentistry and sedation dentistry. Dental Dynamics is credited with the installation of the first and most accurate Cone Beam CT or “CBCT” scan technology of its kind “Prexion”. While I-Cat was introduced few months prior to Prexion, Prexion has been recognized for providing the most accurate 3D images of any CBCT in the industry.

“If a particular dental technology can enhance our ability to make dental procedures more predictable, successful and comfortable for our patients, we will certainly integrate it into our practices. We do not let dental insurance and third party payers dictate our decisions when it comes to patient’s care. Our reputation is built on genuine passion and providing dentistry to those who expect more and the best in quality. CBCT technology allows for much more successful dental implants in Portland. ” Dr Saleh, General Dentist and President of Dental Dynamics PC.

This CBCT technology allows for accurate planning and measurement of dental implants away from nerves, veins, sinuses and other vital structures prior and during the actual surgery. This reduces the complications associated with dental implant surgery.

In addition this new technology paved the way for Computerized Implant Placement Surgery or Computer Guided Implant Placement with DICOM integrating file software. Dentists utilizing computer guided implant placements can place dental implants in Portland and elsewhere, utilizing this technology safely and with fewer complications than traditional methods and planning of dental implants Portland.

Dental Dynamics utilizes Computer Guided Implant Surgery or Computer Guided Dental Placement routinely. Dental implants in Portland are placed at Dental Dynamics in the following sequence:

1- A CT scan is taken on the patient.

2- The CT scan is uploaded to a dental implant placement computer software “ Simplant , Nobel Guide and Implant Logic software are among the most popular”

3- The digital implant placement surgery is performed where the implants are placed with the exact size, depth and length of implant avoiding all vital structures. The software also allows for the planning and simulation of the final restoration of the dental implant.

4- The image is sent to a dental lab where a surgical guide is made from acrylic, with a carefully positioned cylindrical opening that directs the implant drill into similar directions. The depth and size of the implant is placed digitally on the computer.

5- The surgical guide is utilized during the surgery to accurately place the implants. Due to the high degree of accuracy, Flapless “no gum tissue incision “can be utilized in certain cases allowing for faster recovery and less post-operative discomfort after surgery.

6- This technology also allowed for the fabrication of the temporary or permanent restoration to be delivered at the same day. The process is referred to as Teeth-in-a-day which patented by “Nobel Biocare“ a leading dental implant manufacturer.

Dental implants Portland by Dental Dynamics is helping change the way we think about the dental implant surgery process. Implant dentistry in Portland, and the rest of the country, is traditionally focused on utilizing Panoramic X-rays for the planning of the dental implant surgery in the vast majority of cases. In some practices this traditional technology is still being utilized due to the initial high cost investment of integrating Cone Beam CT scan into the dental practice. Panoramic X-rays inherently have 25% inaccuracy in the image which translates to a less predictable outcome especially when multiple dental implant placements are planned simultaneously. The surgery process is changing to a more comfortable and predictable implant placement surgery and ultimately a more successful restorative outcome. Dental Dynamics in Portland has placed safety and accuracy paramount which allows them to guarantee dental implant integration and success. Learn more about this technology by visiting Dental Dynamics. For a limited time they are offering a <font color: "#000000">free consultation that includes the CBCT scan analysis.

Dr.Mo did extensive work, six caps on my upper front teeth, two implants in bottom jaw after removing 7 bottom teeth, design and installation of lower and upper dentures, this guy has bent over backwards to make sure I was happy with the work, which I am.Dr Mo is not the cheapest I have been to,but I probably wouldn’t have had to have all that work done if previous work had been done properly. They have all kinds of little gadgets to distract you when doing the work. I watched two movies on these special glasses while they got those front teeth ready. The office staff made sure all insurances were billed properly and in your favor. All in all a rewarding experience.
